The social worker in this position is responsible for administering the In-Home Aide Program. This work includes determining eligibility for In-Home Aide Services, which includes assignment of the appropriate funding stream to pay for the services; making regular home visits and other contact with consumers and other professionals to ensure the appropriateness of services and to assess changes in situation; managing a waiting list and working closely with the Fiscal Officer to ensure that available funds are expended; communicating with the contracted vendor to ensure appropriate placement of the in home aides and to address problems; and checking monthly invoices to ensure appropriate billing. This position assists citizens with securing placement in assisted living or long-term care facilities when requested and could involve administering the NC Pre-Admission Screening and Resident Review (PASRR). This social worker is also responsible for assessing adults for eligibility for State Adult Day Care/Day Health Funds and monitoring clients in these programs. In addition, the social worker in this position will provide services for other areas relating to adults including Adult Protective Services and Adult Home Specialist duties as needed. This position entails contact with other community professionals and consumers, so strong and effective communication skills, and the ability to resolve conflicts is required, as well as, strong organizational skills and the ability to work independently.
